STUDENT’S STATEMENT OF CANDIDACY

I authorize Young Explorers Abroad to use the above information and my likeness for publicity purposes if I am selected to attend the workshop.

Nominees who have been selected for the workshop will be contacted via email.

Upon successful completion of the program, the student or parent will receive a check payable to U.S. Department of State in the amount of $110 per student who is 16 and older or $80 per student who is under 16, to cover the application cost for a United States Passport. They will also receive a gift card from either CVS or Walgreens to cover the cost of the passport photo.
